Hi there! In response to your query it is highly recommended to check out the central government's notification on changing particulars on PWD certificates. A normal document would not be sufficient here as you would have to actually follow the procedure laid down by the government, get the certificate corrected with the right details and then submit the new certificate instead of the old one.
