My mothers actual name is Manisha ( even on aadhar) but when i was a child and still at home our elders call her Bhagyawati. Due to lack of awareness. In my 10th and 12 th certificate her name is mentioned at Bhagyawati. i want to get it changed. can you suggest how to do it?

You should contact your school and see if you can reapply for your 10th and 12th certificate and tell them that you want to change your mother's name on it. If the school refuses then you can make a affidavit stating that both the name belong to same person.
Since your mother's name is different in her Adhaar and your certificate so it will cause a problem later. So do it as fast as possible.

want to ask, am I only required to have an affidavit which can be used with my documents or should I first create an affidavit and then approach CBSE for change in the marksheet?

First approach CBSE if they agree to give you a new certificate then it's well and good. If they refuse to do so then you can prepare an affidavit from court stating that both name belong to same person. You can later use that affidavit with your documents.
