my name in admit card is Vansh Saurabhbhai Shah and in adhar card is Shah Vansh Saurabhbhai then I have to download an undertaking form or not?

I don't think you have a big difference as on adhar card your surname is written first while in the records of mhcet registration its just your real name written first so that doesn't makes any big difference and will be considered as non objectionable so you don't have to worry for undertaking .

Based on your Query, it has come to my observation that this is just a difference in the writing of the name (First Middle Last or Last First Middle). Both are used in actual practice and in many cases 2nd format is used in Marksheets and other is used in identity proofs.
