For Mathematics you can follow S.N.Dey. It'll help you a lot. Otherwise for good results you may follow:

Rudiments
M.L.Khanna
R.D.Sharma
Arihant text books

Now coming to Physics and Chemistry I would like to suggest you to concentrate on the tutorial notes. Otherwise you can read these books:

H.C.Verma
D.C.Pandey text books by arihant
I.E.Irodov
ABC of Chemistry by modern publishers
O.P.Tandon
Arihant or Dinesh

And one more suggestion go through YouTube educational channels. They will help you a lot.
