Here is the list of documents that are required for Madhya Pradesh NEET PG:

Photocopy of the registration form of a candidate for the selection examination.
Copy of the mark sheet of the selection test.
Original copy of the marks list of the last session of the qualifying examination (MBBS/ BDS).
Original copy of the certificate of passing the screening test from the National Board of Examination (NBE) in the event of qualifying elsewhere from India.
For proof of age, Secondary, Higher Secondary, 10+2 or equivalent examination certificate.
Original copy of local resident (native) certificate of Madhya Pradesh issued by a competent officer.
Certificate of the annual income of the family of the candidate in the year immediately before the examination. This certificate can also be issued by the candidate himself.
Photocopy of Aadhaar card of the candidate.
Original copy of Permanent Caste Certificate issued by a competent officer for those taking advantage of reservation.
Original copy of the disability certificate issued by the Superintendent of Disabled Rehabilitation Centre, Jabalpur, Government of India for the candidates availing the benefit of reservation under the relevant category.
