Here is the list of documents that are required for Madhya Pradesh NEET PG:

• Photocopy of the registration form of a candidate for the selection examination.

• Copy of the mark sheet of the selection test.

• Original copy of the marks list of the last session of the qualifying examination (MBBS/ BDS).

• Original copy of the certificate of passing the screening test from the National Board of Examination (NBE) in the event of qualifying elsewhere from India.

• For proof of age, Secondary, Higher Secondary, 10+2 or equivalent examination certificate.

• Original copy of local resident (native) certificate of Madhya Pradesh issued by a competent officer.

• Certificate of the annual income of the family of the candidate in the year immediately before the examination. This certificate can also be issued by the candidate himself.

• Photocopy of Aadhaar card of the candidate.

• Original copy of Permanent Caste Certificate issued by a competent officer for those taking advantage of reservation.

• Original copy of the disability certificate issued by the Superintendent of Disabled Rehabilitation Centre, Jabalpur, Government of India for the candidates availing the benefit of reservation under the relevant category.
