Hello Naman,
Yes you have to submit Medical Certificate from district hospital approved by CMO of Your district. A Sample format is given in UPSEE Information Bulletin is given at last of that file. You take a photocopy of that format and make a medical certificate according to that. Normally It takes 1 hrs in government hospital. They take few test like Sight Test, Blood Test etc and then they approve it.
