Yes writing any marks of any subject in any application form fillup after 12th class or inter 2nd year you have to write the total marks you have got In theoretical and practical in total. Suppose you are applying for airport cadet service application form. Then to write your marks of physics should be written in theoretical plus practical in total out of 100. Is actually have to write all the subjects marks, which subject has practical works and theoretical writing as well.

You haven't mentioned the exam you're enquiring about, however from your profile it seems you're a medicine aspirant, I assume it's for NEET, in order to appear for NEET candidates are required to fulfill the following criteria;
Attained 17 years by 31st December of the appearing year.
Passed or appearing 10+2 from a recognized board with Physics, Chemistry, Biology/Biotechnology and English as core subjects.
PCB Aggregate inclusive of theory and practical needs to be achieved against one's respective category-
- UR - 50%
- SC/ST/OBC - 40%
- PWD - 45%
As of now there is no upper age limit for NEET till Supreme Court passes any judgments regarding the same hence no more restrictions in number of attempts,
