Since the exam for 1st and 2nd year students will now be held in december 2021 thus we have to fill exam form befor 12 july or we will get another chance to fill the exam form before december exam?

As per your question, I think you are talking about the university exams. Now different universities takes there semester exams in different timeframe and following the guidelines of UGC. Now for the examination you have to fill the form based on what your university asks you to do. If your university wants the form to be filled up in July then just go for it. Don't delay in such situations until and unless you are sure about.
