sir i dont have disability certificate but pwd(yes) written in admit card? now what can i do? plzz tell where can i apply for pwd certificate and how many days i have to wait?

In order to get a unique disability identity card or UDID card follow the following steps
Visit to the official website of UDID
Click on registration link to register with UDID web portal using credentials pwd logs into system and click apply on for disability certificate
Read and fill the online application
Upload coloured passport photo and other requisite documents like income proof, identity proof and sc/st/obc proof if required
Submit data to cmo office/medical authority
Cmo office/ medical office verifies data and assigns concered specialist for assessment
Specialist doctor assess disability of pwd and gives opinion on disability
Medical board reviews the case and assign disability percentage
Cmo office prepares disability certificate and generates UDID and disability certificate.
Data goes for UDID card printing and dispatched to pwd.
