sir I have passed class 12th in 2020 and scored 58% and I am giving again 12th exam in 2021 so what should I opt in jee form that is 12th passed or 12th equivalent.

You need to fill 12th passed in the jee mains form as you have already appeared and passes in class 12th in the year 2020. It doesn't matter if you have applied for improvement examination in the year 2021, you have already appeared and passed the class 12 boards examination.
