Yes, 12th registration form for CBSE do requires your date of birth certificate. You have mentioned that you don't have your date of birth certificate with you so you can also submit your 10th class marksheet of CBSE to the school administrative department as that will be also counted as your date of birth certificate only.
However, it will be better if you submit your date of birth certificate.
