Dear aspirant.
If I'm not wrong you are talking about nativity certificate.
For nativity certificate you need the following documents.
Documents required:
Proof for date of birth such as 10th Passing Certificate, school leaving certificate or hospital record etc.
Proof of any ancestral property in India.
ID Proof with Place of Birth Address (Ration Card/ Driving License/ Voter ID etc.)
Copy of passport (Front and last page).
Signed Authority letter.

- Aadhar Card
- Ration Card
- Birth certificate
- Class 10/ SSLC Certificate
- Parent’s school certificates
- Address proof – (Either a ration card, water bill, electricity bill, or telephone bill)
- Mobile number and email ID is mandatory for online application
- If the applicant has not attained the age of 18, then the nativity certificate of the applicant’s father is mandatory
