I guess you are asking regarding neet examination form filling; yes it is candidates name to be written on your photo. Your photo must have a white background having the name of the candidate i.e your name and date of photograph underneath the image. Size of the file can be 10 KB to 200 KB in JPEG format. When it comes to sign then your sign should be with black ink pen on a white paper and should be scanned to a file with size 4 KB to 30 KB in JPG format.

Hi Shashidhar,
When you are filling up an application form, they will ask you for your recent passport size photographs with your name and date on it. This is a mandatory process for every candidate as you are self attesting your photograph and making it sure that your photograph is clicked on the most recent date. Many entrance exams like JEE have made this process mandatory for every candidate.
