In order to appear for NEET, you need to fulfill the following criteria;
Attained 17 years age on 31st December of appearing year.
Passed 10+2 from a recognized board having Physics, Chemistry, Biology/Biotechnology and English as core subjects, having Biology as an additional also be taken into consideration.
PCB Aggregate inclusive of theory and practical needs to be achieved against one's category-

As of now there is no upper age limit for NEET, hence no restrictions in number of attempts.

you need a combined PCB aggregate, for instance if you belong to general category you needn't have 50% in Physics, 50% in Chemistry and 50% in Biology, you need a combined PCB aggregate of 50% passing in theory as well as practical
