what forms are use for apply NEET

Hello!
You can apply for neet examination online.
It has already been released on 2nd December and the last day to fill your application is 31st of December.
To apply you need to have following requirements:
# scan of photographs in which candidates name and date on which you took photo should be mentioned.
# scan of signature it should be done out of black ink in white paper.
# scan of left thumb it should be done by blue ink on white paper.
# class 10 certificate to be scanned.
You need to carry mentioned id proof in the application to examination hall.
