Don't worry! On JEE Main website in the FAQs, it is mentioned that if candidates who have forgotten to write their name and date below the image, their application forms will not be rejected. Please go to the “Online Application Format” page in the link given below. Also, CBSE will inform the candidates through email or SMS in case there will be any discrepancy in their application forms.
If you wish to, you can send a mail with your rectified image to jeemain@nic.in

If there will be any discrepancy in your image CBSE will send you the message regarding it then you can change your photo you will get edit option.
