When do the preperation for md start ?where?

Since you want to opt for MD, you need to give and qualify the NEET (National Eligibility cum Entrance Test) PG examination. There is, as such no specific time to start the preparation for this exam. You can prepare for it during your MBBS college years or take a drop after the college gets over. The examination will cover all the topics and subjects that you will/have learn(t) during your MBBS. Thus it is pretty simple. All you need to do is be proficient at the subjects taught during this period and you can thus excel in the examination. If you want, you can also join any coaching or take online courses.

M.D. General Medicine is 2 to 3 years a postgraduate course in medicine. The eligibility for which is MBBS degree with minimum aggregate marks as required by the institute.
Average tution fee charged by institutes for MD General Medicine course ranges from INR 10k to 15 Lacs.
