Annexure 9 will be in the business rules in josaa official website. Here is the link for josaa official website.

You can go through this link and check the business rules and the download the annuxure 9 from that website. Fill that annexure 9 and submit to the reporting centres. Annexure 9 is the undertaking by the candidate letter.hope this answer will helps you.

Dear Student.
You have to download the Business Rules file from JoSAA 2019 official website and search for a certain annexure which contains the entire required format for the Medical Certificate. Take a printout and fill all the details correctly and carefully and get it signed by the doctor as well.
