Whose name should be written after signature of father/mother in NEET declaration form of caste certificate?

in the declaration form of obc ncl in neet. whose name will come after fathers signature? will it be fathers name or mine? also can we sign next to the signature of applicant as there is no space above it in the declaration form??

Hello,

After signature of father/mother in NEET declaration form of caste certificate name field is given for candidates name and signature must be put next to the signature of candidate.
