Dear Saurav,
You have to provide the nativity certificate in the format which is provided by the Anna University. This is usually given by the local Tahsildar office.
The nativity certificate requires a sign of VAO (Village Administrative Officer), RI (Revenue Inspector) and Tahsildar.
You will require your ration card to make this certificate.
