You should go for the state counselling (AIQ cut offs will be very high this year) no matter what as you never know when you might get a chance. You should also attend every round of the counselling.

Assuming that you are from the state of Maharashtra (and are willing to take government as well as management seats) here is a list of medical colleges with the cut off for general category for 85% state quota seats (based on 2019 cut offs)-

ACPMMC, Dhule- 501 marks

ARMCHRC, Kumbhari- 490 marks

BKLWRMC, Ratnagiri- 479 marks

BJGMC and Sassoon General Hospital, and College of Nursing, Pune - 604 marks

IIMSR, Jalna- 445 marks

You have a fairly good chance of getting into more than half of the medical colleges i have mentioned, You must definitely attend the counselling.
