There are the some rules for the Thumb impression released by the NTA

1. Put your left thumb impression.

2. Make sure thumb impression is clear.

If you have clear thumb impression no need to worry.

If you haven't given the correct details then your candidature may get cancelled at anytime during the counselling/admission process. You don't need to worry about that, as it will not cause any kind of problem. You just need to carry your passport size photograph for verification and your valid id proof along with your admit card.

i would suggest you not to do any changes to the admit card, it may contain some information that is important,

take the complete admit card to the exam help, if the page won't be helpful, the officials will ask you to remove it. you don't have to

i think proforma is not needed as the information bulletin is not updated......it the same (carbon copy)  as 2019 neet information bulletin. take only what the hall ticket instructions say.
