As you have mentioned that you wrote your surname after the first name in NEET PG 2020 application form but in your certificates the surname is before the first name, I suggest you to edit your application form and write your name as it is mentioned in your

• CA Foundation
• CA Intermediate
• Articleship Training (3 year)
• CA Final

Yes while the correction window is open you do have the option to edit your exam code.
I advise you to keep a check on the correction window and correct the exam code as early as possible!

You will need your 10th pass certificate or atleast marks sheet to upload in the application form. You do not need your 12th certificate. That can wait until counselling. But if you do not have your 10th certificate, then you cannot fill in the application form for NEET.
