As you have 57.40% in RBSE 12th board, you'll be not eligible for AIIMS 2020 as the combined percentage of English+Physics+Chemistry+Biology should 60% for OBC as per eligibility and for NEET, minimum combined percentage of PCB should be 40%. You can improve your marks by giving RBSE Improvement

If you don't have a domicile certificate, it means that you would be applying under All India Quota. 

I see less chances of you getting admission into a government medical college.  However, you must try for a private medical college.

You have a good rank in your category. Considering previous year's data , there was no cutoff of EBC category or infact of BC category for Patna Medical College , Patna. EBC category students were alloted three colleges Darbhanga Medical College , Indra Gandhi Institute of medical sciences

You have not mentioned state you belong to. Anyways with 495 marks, your chances of getting a decent government dental college in your state through state quota(85%) are quite high as cutoffs for dental colleges are usually lower.
