Yup according to the latest public notice released by NTA dated on 12.10.2020, the testing agency is conducting a retest only for those candidates who are unable to attend the exam dated on 13.09.2020 due to mainly two reasons.
1 either they are in containment zone,
2 or are corona

If you are talking about NEET admit card, then Attaching The photo(same as updated at the time of registration), and signature in the admit card is enough for declaration.
There is no need for any other delaration form.

Yes. Because nothing has changed and it is just an exam only for those Candidates who couldnt appear for NEET due to either being tested postive for Corona or were from the containment zones, the admit card with the date September 13th is still valid. Make sure you carry along
Divya, the admit card will still be valid. This is because like JEE, NEET is not beiing conducted in phases. This is the same exam for Candidates who missed attending due to either being tested positive for the virus or were from containment zones. And this is an opportunity exam
