With regards to the question I want to mention that the exact marks that is needed to get admission in AIIMS may vary.It can just be predicted.And according to my prediction its would vary somewhat between 110 to 150 out of 200.

You'll have to fill the place of your residence in the space provided and the date could be any date on or before the date of the actual entrance examination. That is just for namesake, the date won't actually matter. I hope your query has been answered.
