yes you have to appear the NEXT exam for every medical student. The student may be from India or studying aboard and wants to practice in India.
NEXT means National Exit Test. This is a compulsory exam. This is meritous in terms of qualification and recognition.
If you want to go for Post Graduation, then this exam marks will be helpful.

If you consider last year cutoff's then you can easily get various colleges in Tamil Nadu. ThisYear's cutoff's will have a increment of 15-20 marks as compared to last year but still your marks is above all.
With this you can get Coimbatore Medical College, Madurai Medical College, Thanjavur Medical College.
Stanley Will be quite difficult but you can hope for that also.
Based on the previous year cutoff you have chance in the following college,
Raichur Institute of Medical Sciences, Raichur-338
But this year the cutoff will vary based on the difficulty level of exam, no of applicants, no of seats available etc.

As i have mentioned in your previous question as well that we can't predict the cut off with surity as it is practically not possible. Eligibility for Admission to NITs, IIITs and CFTIs participating through Central Seat Allocation Board: Admission to NITs, IIITs, and CFTIs participating though Central Seat AllocationBoard will be based on All India Rank as explained above subject to the condition that the candidate should have secured at least 75% marks in the 12thclass examination, or be in the top 20 percentile in the 12th class examination conducted by the respective Boards.ForSC/ST candidates the qualifying marks would be 65% in the 12th class examination.
Since for Sc category 65% score is must in boards. you are not eligible to get admission using jee mains rank/score.

You can get a northeastern NIT like NIT agartala, mizoram etc. with branches like food processing and you are also eligible for GFTIs. If you want a good NIT then you should take a drop and prepare well for JEE 2020. I hope you have got the point.
