Hi aspirant, since you have 250 marks in your NEET exam, hence you may can take admission in not government but private college for sure.
Also you can take admission in any state as it may even be good and some small concession may be provided if you take admission in different state than yours.

There are very less chance of you getting the Karnataka private medical colleges at this marks. You may get 3-4 private colleges in Karnataka which fees are higher than regular about 16-17lakh/annum.
Colleges with fee about Rs10lakh/annum cut off goes very high. Last year for these College cut off was 590+ marks.
So getting private colleges in Karnataka with regular fees will be minimum 550+ marks. Although it's not fixed,it will be cleared once results declared.

You have your caste certificate in Bihar but domicile certificate in Chattisgarh. Then if you apply for Chattisgarh state quota but you don't use your state caste certificate. You have to apply general category or you have to apply central caste certificate then no problem with be occurs. And yes, you can apply Chattisgarh state quota seats basis of your domicile certificate but don't use your caste certificate. I hope you can understand.

There are very low chances of getting you with CMC, Vellore or CMC, ludhiana.
Both are one of the top colleges in India. CMC, Vellore have only 16 seats open for all india students. Which cut off goes above than 650+
Whereas as CMC ludhiana have only 4s out of 75 seats are oepn for all over India students. So due to very less seat competition is very high. Last year cut off for open category in this college was AIR 16384 ( marks above 605+)
So there are almost no chance of you getting in CMC.
Last year MP state quota cut off for obc category was 555 marks.
So there is probabilities of drop in cut off. So you may have some chances in state quota counselling for getting govt college.
As per the instructions available in NEET UG 2021 information brochure you need to fill the application form by providing the details such as your name, father's name, mother's name and date of birth as per class 10th certificate. SO you have to fill your name as per class 10th certificate record.
