If you sought the appointment under EWS Reservation quota, you have to submit EWS certificate from appropriate authority without any exception. If you fail to submit, you will not be allowed to attend interview at all because documents verification.

Generally for the document verification of your name, your father's name, date of birth, etc. your class 10th result is considered. So if you have used that name during the time of filling of NEET application then it might not be that much of a big problem for you.
