No, the chances of that happening are very low as ETS uses a bank of several sections that differ in difficulty level and questions.
They also periodically rotate/refresh these sections. This is why they have a policy that claims 60 days of gap between two attempts, probably this is done to reduce the odds of a particular student getting the same sections again.
So to answer your question no the question papers are not the same for everyone.

This will depend on cillege cutoff you are applying for.
For top colleges scores above 320-325is must and for TOEFl yiu shall need 110+ scores.
You also need good academic scores and also need to produce SOP and LOr from college to get an admit from top colleges for PHd.
