Yes,The admission into IISC is based on the marks u get in JEE Mains & JE Advanced.
The board sets the cutoff for different exams category and branch wise.So,If you get above the cutoff you have a maximum chance of getting into that college.

IISC admission is sort through national level entrance exams jee main and advanced, NEET and KVPY.Admission is in the basis of ranks obtained in these exams.For admission there is no other aptitude test are available as IISER.You have to get a good score in one of the above mentioned exams to get admission.

First and foremost, let me state that IISc Bangalore is a fine choice of institution, especially if you have a disposition towards a career in scientific research (which is very much in demand).
Coming to eligibility, with 60% in 10+2 and PCM as your subjects, you can opt for the bachelors degree in IISc. Further to that, NEET UG marks and rank will be used for selecting among aspirants for the admission. There is no minimum marks stated by the IISc Bangalore in NEET. I believe the admissions will be done on the basis of merit (based on your marks and rank in NEET as compared to other candidates).
Aside from appearing in NEET 2019, you will also have to fill the online applications for IIScB. The applications will begin from February 2019 onwards. They are likely to remain open till April end of 2019. The application fee would range like this- Rs. 500/- for GN/OBC/KM candidates and Rs. 250/- for SC/ST/PWD categories.

To pursue B.Sc Mathematics in IISc, you should
Pass in +2 level with minimum 60% marks in maths, physics, chemistry as compulsory subjects.
Secure 54%(OBC) in JEE-MAINS 2019.
If you are satisfying the above mentioned criteria, then you can get admission in IISC.

To be honest it will be difficult to get NIT trichy in 76 percentile no matter the PWD reservation. But still you can try the next phase exam and try to improve your percentile.
Now coming to IISC, yes you can Get adm. in IISC without JEE advance. But for that you have to clear kvpy exam.
