Yes, you have to give the GMAT exam or GRE ( depending on your subject) so as to apply for the PhD programs. It is a prerequisite in almost all major universities. Alongwith the score you need to submit a SOP, Documents, recommendation letters and research proposal.

There is no MBBS in United States of America. There you get the degree MD after you complete your undergraduate degree with pre medicine track. Also you need to give MCAT to get admission and not neet. All of these things are different in USA and not similar to India.

In case, you are appearing in 12th then you will have to enter code 1 and if you already have got your marks then code 7 will be applicable.
