You getting good marks dear so can apply any government colleges or private colleges. But it's difficult to predict that which particular college you get. Because the admission rank in every college's differ for students merit list and cut-off marks also.
Your mark's was good but it's totally your choice to what you want. Because if you are stuck in one college for admission it wouldn't beneficial for you i suggest. So you can go for All India counseling because more options are open infront of you.

According to the category rank you have mentioned in the question you cannot get any of the NITs or any of the good GFTIs as per previous year's cut-offs but I would suggest you to sit for some state counselling because in some of the state government colleges you can get admission on such ranks. You can try for some private colleges which take admission on such low ranks but i would recommend you to take a drop for one year and prepare again in the correct way. As you are estimating 154 marks in NEET 2020 out of 720 marks it is quite very difficult to get allotted in the medical college as you have secured 154 marks. And as you belong to ST category there maybe chances but surely we cannot estimate you can definitely get a seat allotted. The expected cutoff marks of neet 2020 in jharkhand.
Candidates who belong to unreserved category required at least 600 to 610 score in all India quota and 575 to 580 score in state quota.
Candidates who belong to OBC category required at least 575 to 580 score in state quota.
Candidates who belong to sc category required at least 505 to 510 score in all India quota and 440-445 score in state quota.
Candidates who belong to ST category required at least 480-485 score in all India quota and 375-380 score in state quota.

Hey dear, if you see last year's cutoff which went very high that even the students getting 500 were unable to get an mbbs seat. So, with this score getting an mbbs seat would be difficult for sure.
