You can apply for NIT jamshedpur using General and home state quota. home state Cutoff for CSE at NIT campus is :-
general male :- 14897 and for general female :- 21630
for All india :- general male :- 9434 and for general female :- 15306
There is huge difference between your rank and previous year cutoff. still it would be better to apply for them. you can get in later round of counselling. you can apply for other branch options or other NIT campus.

It's quite difficult to predict that whether you will get seat or not as cut off marks/rank for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.

*Patliputra Medical college, Dhanbad
Out of these, Rajendra institute of Medical Sciences, Ranchi is the best, you should prefer above Colleges for your counselling

Unfortunately you have little chances or no chances of getting into good colleges. According to the category rank you have mentioned in the question you cannot get any of the NITs/IIITs as per previous year's cut-offs but I would suggest you to sit for some state counselling because in some of the state government colleges you can get admission on such ranks.
Note that the cut-offs are bound to change every year depending upon various factors.

Firstly i would like to appreciate you for your efforts for scoring so much high in one of the most difficult exams of the country .
I can understand that this year despite of scoring such a high score you get such a rank but this shows that how much the competetion is rising and also the cutoff is going to rise so much high .
