Yes a pwd candidate will have to show their medical certificate during the time of document verification - which is a very crucial step during JoSAA counselling, when you physically visit the college. But no, on spot medical checkups are not conducted by the IIT authority but you have to submit a duly filled medical checkup form signed by a registered medicinal practitioner.

The official dates for the JOSAA Counselling 2022 has not been released yet. However, the Online registrations and choice filling for JoSAA 2022 counselling will commence from October 16 for admissions to 23 IITs (Indian Institutes of Technology), 31 NITs (National Institutes of Technology), 23 IIITs (Indian Institutes of Information Technology) and 23 Other-GFTIs (Government Funded Technical Institutes).

It will be really difficult to predict the exact prediction of colleges that you can get as you haven't mentioned your category - OBC, SC, ST, or EWS. This is important because the JEE mains cutoff are different for different category students and it varies every year.
Anyways, if you belong to the OBC category and so with your above mentioned category rank, you have good chances of getting decent NIT's and IIIT's in JOSAA and CSAB counselling in good branches.
But if you belong to any of the EWS, SC, or ST category, then it will be very difficult for you to get any good NIT or IIIT as the required rank for all these categories are around 2k-4k, 2k-3k, 1k-2k respectively in JEE mains, as acocrding to the previous year JEE mains Cutoff analysis.

Your application number and DOB doesn't affect your counselling. The counselling and the seat provided to you depends on the percentile you score in JEE Mains exam. So It's okay if you shared result. the only thing that can happen is that anyone may disturb the colleges you select and they can also check you results and all.

With this percentile, the chances of you getting a decent college are low. Almost all the NITs and IIITs close at 90-92 percentile for general category. You can take an year drop and prepare for next year. If not willing to do so, you may seek admission in your state colleges.

JOSAA counselling process is a national level counselling process. It does not depends from which state you belong. The given below is the eligibility criteria for the JOSAA counselling for the JEE mains exam:
Should have qualified 10+2th (or equivalent) examination with Physics and Mathematics as compulsory subjects along with one of either Chemistry/Biology/Biotechnology/Technical Vocational Subject as optional subject.
It is compulsory for general category candidates to have scored a minimum of 75% aggregate marks for admissions into NITs, IIITs and GFTIs. For reserved categories, the aggregate marks to be secured is 65%. However, if the candidates come under the top 20 percentile of their board, then they will also be eligible for admission.
The JEE Main cutoff should be met by the candidates. However, this does not guarantee admissions.
Now the percentile, so whatever the percentile is you can register for the counselling process. the percentile will help you to get the college if you have good percentile then you might get the good college otherwise you won't get the college. And the rank you mentioned there are less chances of getting the good college.
