Except the allotted institute option you can modify or delete the options filled in the higher priority listed above the allotted institutes. You cannot modify the priority of the option of the allotted institute. Choice 2 indicates that you are satisfied with the current seat allotment but you wish to get seat in a better choice of institute so in second round the options listed above the allotted option will be considered for seat allotment in Second round. For example if CMR University is listed in the 19th option, you can modify or delete all the options from 1-18 and only these options will be considered for seat allotment. If you delete all the options from 1-18 your seat allotment result may not change thus the seat allotted in first round will be retained.
There will be no penalty if you have not paid the fee prescribed by KEA. If you have downloaded the challan and paid the fee but wish to surrender then the fee will be refunded after deducting Rs. 5000. If you do not pay the fee you will not be able to download the admission order further you cannot participate subsequent rounds of the counselling so you will be left with no seat in your hand as you have selected choice 1 after the first seat allotment.
In Karnataka PGCET Counselling choice 2 indicates that you are satisfied with the institute allotted to you but wish to participate in subsequent round so as to get seat allotment in a better choice of institute. Candidates selecting the choice 2 must pay the prescribed fee by downloading the challan and download the admission order and report at the allotted institute. The fee that you have paid will be transferred to the institute in which you are allotted seat in the first round hence you can see the name of the institute allotted in first round of the counselling. If a seat is allotted in second round of the counselling the previous seat allotment will gets cancelled automatically and you do not have to pay the fee once again. If no seat is allotted to you then your seat allotted in first round will be retained.
Yes, the candidates who have selected choice 3 after first seat allotment will have all the list of options that they have selected while filling the choice filling form in first round displayed on the scree. They can also modify or delete the options that they have filled in the first round of the counselling. So you will be able to modify or delete the options in the choice filling form. Ensure that you select only the institutes better than the allotted seat and the colleges in which you are interested to seek admission.
There is no provision to claim the seat allotted in first round Karnataka NEET UG 2021 if you haven't exercised any choice after first seat allotment. You cannot participate in subsequent rounds as the candidates who haven't exercise any choice in first round are not eligible to participate. If you wish to seek admission in this academic year then you may have to seek admission in any other course or you have to appear for NEET UG 2022 if you wish to seek admission to MBBS/BDS. See the last of registration and payment of the fees for the KEA NEET 2021 was extended till 27th December,2021. At this moment, there are no such information available regarding the extension of the period again. So technically, it will not release the registration. Although, keep an eye on the official website, if by chance KEA reopens the registration, but the probability is low.

There is no specific instruction whether your document should be self attested or attested by gazetted officer.

You have not mentioned your category, so it is quite difficult to provide precise information. Some colleges give admission on the basis of board percentage whereas some also consider NEET marks. But, in Karnataka students who scored minimum 40% in Physics, Chemistry and Biology marks in 10+2 exam are eligible for admission.
Considering that you are a general candidate, 85% is quite decent score for getting a seat in good colleges in Karnataka and Bangalore through counseling.
