The cutoff marks change every year. It's depends upon number of students that appeared in examination, number of seats available in colleges, difficulties of question paper etc.
So the last year neet 2019 cutoff for Madhya pradesh is 530 to 600 marks for general category candidates. And for candidates who belong to SC catagory required 490 to 510 marks and candidates who belong to ST catagory required at least 475 to 500 marks for government medical college.

Going by last year's cutoffs and your score, it would be difficult to get a seat even in private Medical colleges with this score as last year the minimum cutoff score was 324 for SC Category and the respective college of Amaltas Institute of Medical Sciences while Index Medical College had the cutoff score of 328 and 331 for LN Medical College for Sc Category. As per previous year data it's tough to secure a GMC seat under state quota with your given marks, the reason being last year 530 marks secured a seat in GMC Shivpuri for UR, hence your marks are comparatively less, you can get private colleges like Amaltas Institute of Medical Sciences, Index Medical College etc. This is purely based on previous year data, cut-offs are subjective to change every year based on multiple factors like seat matrix, marks obtained in exam, toughness level of paper, total number of candidates participating in the exam etc.

It's quite difficult to predict that whether you will get seat or not as cut off marks for medical colleges keeps changing every year depending upon various factors like number of students that appeared in the exam, kind of marks obtained by them , toughness level of the examination etc.
But when we go through previous data then it suggests that with 591 marks you hold a very good chance in a Government Medical College
For example, in neet 2019, 525 marks for obc category got a seat in Government Medical College, Shivpuri under MP state quota
Since you have not mentioned your category so considering it as general.
So on such marks and category you have very low chances of getting Private medical colleges in Madhya Pradesh according to previous year's cut-off.
