Guessing you from general category.
There's no minimum percentage requirement for each subject individually, it's for aggregate 12th marks i. e at least 75 % in aggregate of 5 subjects including physics, Chemistry, maths, one language and one any other subject other than these.

The chapter-wise weightage is only a reference as to how important the chapters are and the base they form for other chapters too. Questions can be asked from other chapters too, that too, 2-3 questions even if they have a low weightage.
