There are no such problems expected. Now if any problem occurs then you can always give a counter argument that you have changed your permanent to the present address as a result two address differs. But in general no such problems occurs.

There are no colleges which require separate filling of forms for NEET counselling.
For admission in private colleges you have to go through the counselling conducted by MCI in a separate section of DEEMED universities.

The FMGE 2021 test date for December session has been published by the National Board of Examinations (NBE), which is December 12, 2021. For applicants who complete the application form before deadline, the NBE will provide the FMGE 2021 hall ticket. The FMGE screening test 2021 will be carried out in two stages, part A and part B, on computer. Foreign graduates who want to undertake the screening test for a registration certificate from the National Medical Commission (NMC) must know the FMGE essential dates for the Decade 2021.
FMGE 2021's application form for the December sessions will be available online. Aspirants should confirm their eligibility before registering for FMGE 2021, as only those who meet the eligibility requirements are eligible to apply for the screening exam.

MCI (Medical Council of India) also known as FMGE (Foreign Medical Graduates Examination). For the eligibility to write MCI exam, the candidate should be a citizen of India or overseas citizen of India and must have the primary medical qualification which is approved by the Indian Embassy concerned, to be a recognised qualification for enrolment as a medical practitioner in India. And the candidate who hold an UG medial qualification from Australia/ Canada/ New Zealand/ UK/ USA and seeking provincial or permanent registration are exempted to give the screening test. Contact your college authority as soon as possible. This may create problems in multiple cases like verification of documents during Job/applying for a course. So you shouldn't encourage any discrepancies. If you have correct spelling in your marksheet and incorrect in MCI certificate then it can be a changed by filing up forms and giving affidavit with all the proofs. If overall you have spelling mistakes then also the procedure will be the same.
**See the main thing is you have earned the degree so why will you encourage any mistake? May be you won't face any problem in near future but in sometime else if you face problem then it will be a chaotic experience for you. So contact your college authority and get the norms to fix the mistake.

To apply for MCI eleigibility certificate, it is necessary to have 11th and 12th class marksheet. You can attach 12th class marksheet which you get from the boards. And for 11th class marksheet, you can attach the scorecard which you get from school from where you have pursued 11th class. The new rule of MCI is the candidates are allowed , without neet.and it is one time exemption.
MCI conveyed this in the view of covid 19 pandemic all students need not to take neet .
However these candidates would have to appear later and qualify the exam as per regulations of 2002.
The exemption is one time measure in view of covid 19 scenario.
