Fill it through a doctor.

Clearly write your rank and name, and then provide the details like height weight BMI chest size and others to the doctor. They will check for eyesight and colour blindness and after checking the details, the examining doctor will sign the certificate.

How much marks needed by general candidate to get a seat in government medical college?

It is variable each year. However as a general idea, try to score above 540 for a government MBBS seat in state quota and above 580 for the same in all India quota. Aim for as high as you can.

MY neet 2018 percentile is 70% ,AAA+ in category UR how much will my MBBS seats cost ?

With the score of 196, you won't be able to get government or even private MBBS seats. Government BDS as well as AYUSH seats are also non gettable at this score. You may try for private AYUSH courses though.
