Hello Titli, If you are having any physical disabilities, then, Yes, you need to provide your medical certificate during the document verification process. Because getting admission under PwD quota required the candidate to produce medical certificate during the time of admission.
You can take up tests like ARS/NET, Indian Institute of Space Research and Technology (IIST) entrance test or scientist test by DRDO.
