You will have to qualify NEET to be eligible for admission in ESIC. Once you qualified NEET you will be subjected to further procedures. You can also go through the website of ESIC

State domicile can be identified only with your Aadhaar card or family ration card. I think that you would have you address as Uttar Pradesh in both the cards. So you will be able to apply for state domicile in Uttar Pradesh only and not in Delhi. Studying
