Yes, you are eligible to appear for NEET UG 2020.And your code will be 2.Code 2 says that a candidate who have studied for a period of 12 years appeared for higer secondary examination or the Indian School Certificate Examination (ISCE) or its equivalent must opt for code 2. You must have studied 10+2 with Biology/Bio-technology, Physics, Chemistry and English.

Code 02 says: The Higher/Senior Secondary Examination or the Indian School Certificate Examination which is equivalent to 10+2 Higher/Senior Secondary Examination after a period of 12 years study, the last two years of such study comprising of Physics, Chemistry, Biology/Bio-technology (which shall include practical tests in these subjects) and Mathematics or any other elective subject with English at a level not less than the core course for English as prescribed by the National Council of Educational Research and Training after introduction of the 10+2+3 educational structure as recommended by the National Committee on Education.

It also offers the course of MD in Microbiology where entrance is through NEET PG. • And regarding 75% in 12th, if you have secured a minimum aggregate of 50% in Physics, Chemistry and Biology, you are eligible for NEET. It is 50% for General Category, 45% for PWD and 40% for OBC,SC,ST. Though you are a Post graduate, because the basic eligibility criteria for appearing in NEET is 12th, only 12th aggregate is considered. So in 12th in Physics, Chemistry and Biology your score should be 50%.
• If you have had atleast 2 subjects among Physics, Chemistry and Biology in your Bachelor's degree, your code will be 06. Code 06 says: B.Sc. Examination of an Indian University provided that he/she has passed the B.Sc. Examination with not less than two of the subjects Physics, Chemistry, Biology (Botany, Zoology)/Biotechnology and further that he/ she has passed the earlier qualifying examination with Physics, Chemistry, Biology and English. This is the most apt code for you.
