what does it mean "Affix a recent color Photograph and get attested by a principal of the college where studied" in APICET. is this mandatory

'Affix a recent color photograph' means attach your current coloured photograph which must be in the required format and 'get attested by principal of college where you study' means you have to get it signed by your principal to proof that this is genuine.
Yes this is mendatory process to give evidence.

i will write EAMCET exam for second time this year , is principals signature needed on Application form?

Hello,

You can get online filled application form attested by your college's Principal or even a Gazetted Officer attestation is accepted. So, you can get whichever is convenient to you. You need to submit the online filled application form to the invigilator on the day on exam and you must affix your passport passport photo.
