Neet application form is published in 10 languages those are Bengali, Hindi, English, Tamil Telugu, Kannada, Orya, Marathi, Gujarati and  Assamese. In which you have read that question will be in Tamil language medium then once submitted application form will not be changed of languages which has been introduced in the form in any case. But generally if your medium is English too then your paper can be in bilingual mode means you will get a option to see the question paper in English also. So nothing to worry.
