Hope you are doing great !! I want to tell you that your chances of clearing the JEE Advanced with 50 marks in SC/St category is very very less . Therefore , the chances of getting admission in any one of the IITs is quite impossible . Hope it helps and all the best for your future endeavors . For the iit you need to have at least 95+ marks for reserved Categories .

Hope you are doing well ! No there is not any kind of reservation however As per Government of India requirement, 27% of the seats are reserved for NC-OBC, 15% for SC, 7.5% for ST candidates, 5% for Persons with Benchmark Disabilities (PwD), and up to 10% for Economically Weaker Sections (EWS) ..

See for NEET UG different categories have been provided reservation quota as per the norms of Indian Constitution and for Physically disabled candidates there is horizontal reservation of 5% across all other categories.
Here horizontal reservation means PwD category gets reserved along with other categories as well like there is 27% reserved quota for OBC-NCL and out of that 27%, 5% seats will be reserved for PwD candidates and those candidates are to be called as OBC-NCL PwD candidates, and likewise with other categories as well.

Candidates who qualify as physically disabled must have a current certificate proving their disability.
For candidates with physical disabilities, there is a 3% horizontal reservation in the 15% All India UG Quota seat. Candidates with a locomotor handicap in the lower limb that ranges from 50% to 70% are eligible for this reservation.
However, if there are still open seats under the 3 percent limit, consideration will be given to those who have lower limb locomotor disabilities that range from 40 to 50 percent.

With 402 marks it will be difficult to get admission in government medical college. As you belong to reserved category you have a chance of getting admission in private medical colleges. You will be also having fee reduction in private medical colleges. SC candidate should score 490+ to get a seat in government medical college.
