You can opt for Private Medical Colleges as there are plenty of them
With a category rank of 4227, you are eligible for seat in NIT Uttarakhand, Sikkim and Manipur in Civil, EEE and CSE fields.
I believe if there are no vacant seats after 6th Round in ECE at IIIT Allahabad, you will not make into it, in spite of the fact that you have a rank very near the 6th Round Cutoff in Respective Category.

I believe at a rank of 246 in TS PGECET in ECE Branch, you have fair chances in securing a seat ar Osmania University, JNTU Hyderabad, Kakinada or Ananthapur. All the universities have excellent ECE Departments with good infrastructure and faculty.
