Actually there is only one Central e w S certificate which is valid for all states in India. However for Mp they have some changes in the criteria of e w S certificate in their state. But as you have Uttar Pradesh w certificate it is also valid

No, JEE Mains will not accept Category Certificates from State. Such certificates are only used in State counselling. For JEE Mains, you need a central category certificate.

See there are various medical and dental colleges in UP which accepts admission on the basis of NEET score and different institution have different cut-offs for different categories on the basis of NEET scores and because you have not mentioned the name of institution so it's quite difficult
In 2019, under State Quota, 575 marks was the cut off for a seat in GMC. Now this is for General Category. EWS also had pretty much the same cut off. And now this year it has been little tough with the top NEET score being 720 out of 720.
