According to the preparatory rank and category mentioned by you in the question you have almost no chances of getting into any of the IITs.
Note that the above prediction is made on the basis of previous year's cut-off and the cut-offs are bound to change every year depending upon various factors such as the number of candidates appearing in an examination, the number of candidates qualifying an examination, difficulty level of the paper, and so on.

For college:
To get a seat in a government medical college, you need to get around 550-600 out of 720 (for general category student). Hence you should target to score above 600 to get a seat via All India 15% quota. Cut off marks for state quota varies per state but still you need to fetch more than 550 marks. The real competition lies between 600-650. This is where you will be securing a seat in a medical college but your marks will decide your college. Above 650, you get all good colleges. If you are aspiring for MAMC,UCMS, KEM, VMMC, LHMC etc - you will be needing at least 650 marks. And for your dream destination AIIMS, New Delhi - you will need to score 675 marks.

You mentioned that you secured with a rank of 20656 in AP EAMCET 2020 as a BC-E candidate. No, you're not eligible to get a free seat in D. Pharm course. Since you've mentioned clearly your category, so maybe you can't be able to get a seat in the government college offering D. Pharm. See, it will be pretty difficult to get a seat in the government colleges but there are private institutes where you can get your admission in the course of your choice. In this case Firstly you can consult your seniors or college administration about the stream change because in many colleges it is allowed to change your stream in second year based on the marks of first year
Secondly you can prepare side by side for coding all types of languages(I would suggest) and many basics of the CSE and prepare for your placements as company would not hesitate in taking a ECE student having knowledge of CSE because they will think why to pay for two different persons for different works if the works are done by a single person.
You can also appear for CSE branch in GATE exam because rules are changed that you can give one more stream exam other than main stream then bring good marks and get placed.
