You will be applying for NEET as a UP domicile candidate who is currently residing in Jammu and Kashmir. I am assuming you have also completed your schooling from J&K or will be doing so this year.
As such, I would recommend you keep the following points in mind:

As per the NEET rules, candidates who have cleared their Class XII exam or are going to appear for Class XII in Jammu and Kashmir and who will also be appearing for NEET at an exam centre located in Jammu and Kashmir need not enter their Aadhar number while filling the NEET Application Form. They can use alternative identity details like passport number, ration card number, bank account number or any other valid Government identity number. Therefore you will not need Aadhar number while filling the NEET application form as long as you enter exam centres located in Jammu and Kashmir in the form.
The address that you enter in the NEET application form does not have any bearing on the states you will be eligible to apply for. This is because you will have to fill a separate application form when applying to colleges in the state of your choice and it is the address you enter in that which will be taken into consideration. That said, the NEET application form has a field asking for ‘State of Eligibility (for 15% All India Quota)’. Ensure that you select Uttar Pradesh here.
In the Residential address column, enter the address you are currently residing at in Jammu and Kashmir.
As per the NEET regulations, candidates who are from Jammu and Kashmir but wish to be eligible for seats in 15% All India Quota (AIQ) should submit the Self-Declaration form included as part of the NEET application. While you have already entered your state of eligibility as Uttar Pradesh in the step mentioned above, you should also submit the Self-Declaration form. Note that this will make you ineligible for State Quota seats in Jammu and Kashmir colleges but eligible for All India Quota seats. This will also not affect your eligibility in other states.
After you have appeared for NEET and cleared the same, wait till the Directorate of Medical Education and Training (DMET), Uttar Pradesh, releases the application form for State Quota admissions in Uttar Pradesh. As per the Uttar Pradesh eligibility criteria set by the DMET, you will be required to produce an ‘Original Resident Certificate’ in the format given in the UP NEET Prospectus when applying for admissions since you have not been residing in UP but are still a domicile of the state. Also submit an address located in Uttar Pradesh and produce your Original Resident Certificate confirming this address at the time of document verification.
By keeping in mind the above steps, you will be able to apply for admissions in Uttar Pradesh despite appearing for NEET in Jammu and Kashmir. I hope this clears all your doubts but feel free to ask if you have any other questions.

In order to apply for NEET Application form you should have Aadhar card.
Only students who belong from Assam, Meghalaya and Jammu & Kashmir can apply through Ration Card Number, Bank Account Number, Passport Number or other Government ID card.
